ADH Application -
FOSTER HOME LICENSE, ADULT DEVELOPEMENTAL HOME LICENSE APPLICATION
For a Foster Home License or Adult Developmental Home License each
applicant must complete a separate application unless legally married;
married couples apply jointly. For Adoption Certification, married
applicants apply jointly; if unmarried, only one person in a couple may
apply for certification.

ADULT DEVELOPMENTAL HOME CAREGIVER ASSESSMENT GUIDE
The Guide is a tool used by DES to assess your skills, experiences, stability, motivation, and other factors as they relate to providing care for an adult with developmental disabilities. The assessment is intended to promote discussion and an exchange of information between you and the licensing specialist. The goal of this information exchange is two-fold:
- To assist you in learning about your abilities to provide care to an adult with developmental disabilities, and
- To assist the specialist in making recommendations regarding your application.
Completion of the Assessment Guide is necessary to assist the licensing specialist in writing your Home Study. The information you provide during the assessment process will only be used by DES to evaluate you for licensure.

Certification Record Self Disclosure -
CERTIFICATION RECORD SELF-DISCLOSURE
Your fingerprints will be submitted to the Arizona Department of Public Safety (DPS) and the Federal Bureau of Investigation (FBI) for criminal history check. Your self-disclosure on this affidavit and the information provided by your criminal history check will be used, as authorized by Public Law and Arizona Revised Statues, to help us determine your fitness to have unsupervised access to vulnerable persons.
CPS Clearance -
ADOPTIVE FAMILIES CENTRAL REGISTRY RECORDS CLEARANCE
Child Protective Services (CPS) records are confidential and can be released only to those individuals permitted by state and federal law. This form is to be completed for all household members. The requested information will be used to check the Child Protective Services Central Registry for any history of prior reports.
