What are the requirements to become licensed as a developmental home provider?
- At least 21 years of age
- Married or single
- Legal United States and Arizona resident
- Extra room in your home
- Adequate transportation
- Have drivers license, vehicle registration and proof of insurance
- Pass a home inspection
- Pool fenced
- Proof of financial stability
- Three references to verify a reputable and honest character
- Adult household members must be fingerprinted for a criminal history records check
- Receive a physicians statement verifying emotional, mental and physical abilities
- Must be knowledgeable, stable, mature, and have nurturing skills
- Completion of all training classes including CPR, First Aid, Article 9 and PS MAPP for child homes
- Complete a family home study consisting of individual and family interviews
